TRANSFER OF TELEPHONE IN CASE OF NEAR RELATIVE

INSTRUCTIONS:

1. Please make zerox copy or type the form in the received on fax on good quality paper prefereably bond paper and fill in the application neatly.

2. Application form for transfer of telephone should be signed by the person in whose name the telephone is working.

3. Filled in application form should be submitted at Area Customer Service Centre in whose jurisdiction the telephone is presently working

4. Please submit the following documents:

a) Application form.

b) Indemnity Bond from the transferrer and transferee.

c) Proof of relationship.

d) Duplicate PAF from transferee.
